SeGOV User Registration and Change Forms
Registration and Changes can take up to six weeks to process. If your program office has a firewall, the department needs to
know the specific ip the Department will see when you log in (please see Training Pre-Requisites). The information requested
on these forms is for your use so you can prepare for what information is needed by the Department in order to gain access.
There is a maximum of two users per county office, a primary and a secondary user. Please be sure to complete your form entirely.
Forms should be sent to the MATP Inbox, please be sure to cc your
County’s Program Advisor.
